If you would like to support Lifeline of Ohio through an event or other fundraising opportunity, we have resources to help! We may be able to provide:

  • Giveaway items or raffle prizes
  • Educational materials or a presentation about donation
  • A Donor Designation Station
  • Approved use of the Lifeline of Ohio logo

To apply for support for your fundraising event, please complete this form and email the completed copy to development@lifelineofohio.org

Once we’ve received your form, you will be notified by our development team about the status of your application. To best support your event, we ask that this form be completed and submitted at least 30 days prior to your scheduled event.

Your generous financial contributions will help us provide bereavement and aftercare services to the families of our organ, eye and tissue donors; reach out to the community about donation; and support general operations of the organization. You can be confident your philanthropic gifts are used with compassionate consideration to best serve those in need. Here are just a portion of the ways your financial contributions help.

Bereavement Program We support our donor families with a two-year comprehensive support program, including grief materials, monthly mailings, events and legacy-building opportunities.
Lifeline of Ohio’s Donor Memorial This public Memorial honors Lifeline of Ohio’s organ, eye and tissue donors. It is the only one of its kind in Ohio to remember, honor and give thanks and gratitude for the gifts of life, sight and healing. We ensure the Memorial is beautifully maintained and engrave the names of new donors twice a year.
Shawls of Support Program This program honors local organ, eye and tissue donors by providing their families with the gift of a handmade shawl to comfort them in their time of loss. The shawls are made by Lifeline of Ohio Donate Life Ambassadors including transplant recipients, donor family members and people in the community.
Educational Outreach Lifeline of Ohio provides donation education in high schools, vocational and career centers, colleges and universities, driving school courses, BMVs, workplaces, faith organizations and to community groups.
Multicultural Outreach We participate in events such as the African American Male Wellness Walk and the Linden Community Festival & Expo, provide outreach and donation education to senior citizens, support and outreach to faith communities and provide information and outreach to the LGBTQIA+ community.
Community Outreach Lifeline of Ohio works with community partners, including the Ohio BMV and more than 70 hospitals in Central and Southeast Ohio and West Virginia, to support and facilitate organ, eye and tissue donation.
